HIQR/HOQR/Annual Payment Update
QualityNet ( www.qualitynet.org) is the "go to" source for the Hospital Inpatient Quality Reporting (HIQR) and the Hospital Outpatient Quality Reporting (HOQR) programs. Hospitals are encouraged to visit this site frequently for HIQR and HOQR Program requirements, including Annual Payment Update information for both programs. To access relevant sections for each program, hover over “Hospital-Inpatient” or “Hospital-Outpatient," select a dropdown box and review the content.
One component of the Hospital Inpatient Quality Reporting (HIQR) and Hospital Outpatient Quality Reporting (HOQR) Programs is maintaining an active QualityNet Security Administrator. One active QualityNet Security Administrator is required, however, CMS recommends registration of two QualityNet Security Administrators. The current registration forms are posted below.

My QualityNet is a secure Web-based venue that allows the exchange of data files that contain Protected Health Information (PHI) with the Center for Medicare & Medicaid Services (CMS) and eQhealth Solutions and between your hospital and CMS and/or eQHealth Solutions. My QualityNet sign-in is accessible on the top of every page on www.qualitynet.org. This secure section of QualityNet also contains reports related to clinical data submission for HIQR and HOQR, such as the “APU Dashboard," the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) data submission reports, HOP QDRP Reports and OPPS Clinical Warehouse Feedback Reports.
Hospital Specific Reports such as, but not limited to, 30 day AMI, HF, PN Mortality and Readmission reports are sent to your MyQualityNet > Exchange Files > Send/Receive box. Hospital Quality Alliance (HQA) Preview Reports are accessible only via MyQualityNet. Again, MyQualityNet is the only CMS-approved method for the electronic transmission of any documents containing Protected Health Information (PHI). Reminder: To maintain an active QualityNet Security Administrator account, sign-in to MyQualityNet at least monthly.

Recommendation: If there is a change in QualityNet Security Administrator(s) at your hospital, ensure a process is in place to register another QualityNet Security Administrator so as to maintain at least one, preferably two, active QualityNet Security Administrator account(s).
