The Patient Safety and Clinical Pharmacy Services Collaborative was honored with the Pinnacle Award for Voluntary Health Agencies, Non-profit Organizations, Associations, Government Agencies and Public/Private Partnerships. This video describes PSPC's push toward empowering pharmacies to improve patient care and assist patients and their caregivers in achieving better outcomes from their medications.

(PSPC)
The Health Resources and Services Administration (HRSA) and the Centers for Medicare & Medicaid Services (CMS) have joined together in a national quality improvement effort to improve health outcomes and patient safety by curbing adverse drug events in seniors, a major source of potentially preventable patient harm. 

Community teams are being formed now.
Multi-disciplinary teams will work across settings supporting high risk patients. These teams will include representation from pharmacy, hospital, nursing home, primary care and patient advocacy settings.
