Therefore, eQHealth only collects personal information if specifically and knowingly provided by you.

We DO NOT sell or share your personal information. At anytime you may opt-out (have the option to stop receiving) e-mail updates from eQHealth. Simply send an e-mail to webinfo@eqhs.org asking to be removed from the distribution list.

Sending Email
Please remember, e-mail messages are NOT secure. Never send any information in an e-mail that you would not want others to read. E-mail messages are sent from computer to computer and have nothing to do with the actual Web site you may be visiting.

Please DO NOT send personal information such as Medicare or Social Security numbers, medical history or any other personal information that you do not want others to see. It is best to send sensitive information through the postal mail system.
